Care Finder

The Care Finders program provides support to vulnerable older people, including those who are homeless or at risk of homelessness, to interact with the My Aged Care system, access aged care services and be linked in with other relevant supports.

Based in North Melbourne at the Inner Melbourne Community Hub we provide both centre-based and outreach support to the Northern and Western suburbs of Melbourne.

The regions comprise the LGA’s of Melbourne, Yarra, Darebin, Moonee Valley, Merri-bek, Hume and Macedon Ranges.

Eligibility

  • Older people and people who have aged prematurely; aged 50 years+ or aged 45+ for Aboriginal and Torres Strait Islander People.
  • People needing support navigating the Aged Care system and needing support to be linked in with appropriate Aged Care and community-based supports to manage their health, wellbeing and independence.
  • Live in the Northern suburbs of Melbourne.
  • People with no existing supports (including family, carer, worker) who would be otherwise unable to navigate the Aged Care system and would be at risk of falling through the cracks.

Referral

Clients can self refer, or a worker can make a referral on their behalf.

Regardless it must be done via one of the following channels:

  • Email carefinder@vincentcare.org.au to receive a referral form to complete.
  • You can contact us via phone and speak with a worker.
  • Drop in to our Homeless Resource Centre, Monday to Friday 9am-1pm.
